We had some instances of a failing upgrade, along with error’s thrown as Upgrade Failed.
We are not entirely sure what was failing albeit we had the log captured, but running the following after the failed upgrade resolved and recovered along with repairing to the most recent build.
This also seems to be the goto when there is trouble with Discourse
cd /var/discourse
git pull
./launcher rebuild app
